IPHCA’s SIMIS Network was initially implemented in phases. Following is the current status of the implementation of the i-net network in Illinois:

Phase I

  • Christopher Rural Health Planning Corporation, Christopher, Illinois
  • Community Health Emergencies Services Inc., Cairo, Illinois
  • Community Health Improvement Center, Decatur, Illinois
  • Heartland Health Outreach, Chicago, Illinois
  • Heartland International Health Center, Chicago, Illinois
  • Infant Welfare Society, Chicago, Illinois
  • Rural Health Inc., Anna, Illinois
  • Southern Illinois Health Care Foundation, East St. Louis, Illinois

There were eight Phase I agencies, which represented 49 sites and well over 110,000 patients.  Implementation began in January 2002 and was completed in September 2003.

Phase II

  • Asian Human Services, Chicago, Illinois
  • Aunt Martha’s Youth Service Center Inc., Suburban Chicago, Illinois
  • Central Counties Health Centers Inc., Springfield, Illinois
  • Chicago Family Health Center, Chicago, Illinois
  • Circle Family Healthcare Network, Chicago, Illinois
  • Heartland Community Health Clinic, Peoria, Illinois
  • Shawnee Health Services, Carterville, Illinois

The implementation process for Phase II sites began in October 2003, and all seven Phase II agencies (which represented an additional 20 sites) were live on the system by October 2005. At the completion of the Phase II implementation, the i-net network was hosting 69 sites which served approximately 200,000 patients annually.

Phase III

The implementation process for Phase III sites began in the spring of 2006.  Currently, there are 4 agencies that are part of the Phase III implementation.  They are:

  • Beloved Community Family Wellness Center, Chicago, Illinois
  • Cass County Community Health Clinic, Virginia, Illinois
  • Centro de Salud Esperanza, Chicago, Illinois
  • Whiteside County Community Health Center, Rock Falls, Illinois
